Cook County Health has reopened 3 mass vaccination sites in order to help vaccinate more individuals against COVID-19. Demand for the vaccine has been on the rise with the Omicron variant surge, and Cook County hopes that the return of these community sites will help those who are having difficulty making appointments for the vaccine at a doctor’s office or local pharmacy.
Any individual 5 and older can be vaccinated at one of the three locations below. Each site offers the Pfizer, Moderna, and Johnson & Johnson vaccine and individuals can receive their first, second, or booster shot as well. Walk-ins are welcome, but it is recommended to call and schedule an appointment at (833) 308-1988.
- Forest Park – 7600 Roosevelt Road, Forest Park, IL 60130
- Matteson – 4647 Promenade Way, Matteson, IL 60443
- Des Plaines – 1155 E. Oakton Street, Des Plaines, IL 60018
